A 58-year-old male with a diagnosis of giant cell arteritis (GCA) was admitted to our hospital because of a high level of vascular inflammation biomarkers. Laboratory examination revealed an elevated erythrocyte sedimentation rate (66 mm/hour), an elevated C-reactive protein level (21.2 mg/dL), a positive antinuclear antibody (1:320), a positive myeloperoxidase antineutrophil cytoplasmic antibody, and a positive C-reactive protein and antinuclear antibody. A weak positive beta 2 glycoprotein-1 antibody was also detected. A skin biopsy of the lower leg showed vasculitis. A knee joint biopsy showed a massive lymphocytic infiltration and multiple giant cells. A histological examination of the knee joint biopsy revealed vasculitis of the small vessels, mainly affecting the synovial membrane, adipose tissue, and muscle. An immunofluorescence study of the skin biopsy revealed antinuclear antibodies and antiphospholipid antibodies. The patient was diagnosed with systemic lupus erythematosus with a high thrombotic risk and was treated with prednisolone (60 mg/day) and intravenous methylprednisolone pulse therapy (1000 mg/day for 3 days) followed by oral prednisolone (60 mg/day). The prednisolone was gradually tapered, and the inflammatory markers gradually decreased. After the first pulse therapy, the levels of erythrocyte sedimentation rate and C-reactive protein became normal. The concentrations of antiphospholipid antibodies in the patient's serum decreased from 73.1 to 1.6.
